As Parkinson Partners continues to grow, the leadership and board of directors believe that it is important to get feedback from the community we serve. We want to know what you think about support group meetings, speakers, donations etc. To that end, we have created a Needs Assessment Survey asking a variety of questions. We would appreciate your time in taking the survey and giving us your thoughtful responses. Your answers will be considered when the board sets agendas, goals and identifies the needs for the organization and the greater PD community.

The link to the survey is below.  Please complete it whether you have PD, are a family member, or have a connection to the PD community. Couples are encouraged to take the survey separately so both responses can be recorded.

Those outside of Erie County are reminded that this survey is for you as well. Your responses will help us plan for support groups and programming in your area so let your voice be heard.
